Post-acute EHR integration

AGIMAN drafts the visit and hands it to MatrixCare

AGIMAN captures the encounter in the room, drafts the note, and suggests ICD and CPT codes on an appliance inside your facility network, then maps the signed result into MatrixCare. A clinician reviews and signs before anything reaches the resident chart.

  • HIPAA compliant
  • Runs on your network
  • Clinician signs every note

The connection

How AGIMAN connects to MatrixCare.

What MatrixCare is

MatrixCare, a ResMed company, is an EHR and revenue cycle system for out-of-hospital care: skilled nursing, senior living, life plan communities, home health, and hospice. It carries the documentation those settings depend on, including MDS and OASIS assessments, eMAR, physician orders, and care plans. It is run by post-acute operators and the nurses, aides, and clinicians who chart at the point of care, not by a hospital department.

The access path

FHIR R4CCDAHL7v2OAuth 2.0

AGIMAN connects through the access MatrixCare approves for integrators, using FHIR R4 and CCDA where they are available and HL7v2 messaging where the workflow runs on it. MatrixCare reviews third-party integrations through its Connections program and Partner Marketplace, and the first export path is scoped to the documents and codes your facility actually charts. We test that path against your own MatrixCare instance before it is turned on for go-live, so the mapping matches your fields and your setup, not a generic template. Nothing about the draft leaves the appliance during capture and drafting.

The MatrixCare export path is scoped to your documents and codes and tested against your own MatrixCare instance before it is turned on for go-live.

The workflow

From the exam room to MatrixCare, with you in control.

1

Capture

Record the visit in the room

AGIMINION captures the encounter audio in the resident's room or on the home visit. The audio and transcript stay on the appliance inside your network, with no internet connection needed to transcribe.

2

Draft

Note and codes, in the clinician's style

AGIMAN drafts the chart note in the clinician's own style and suggests ICD and CPT codes for the encounter. It can also prepare a referral letter, an intake or visit form, and the follow-up task and schedule note.

3

Review and sign

Clinician reviews and signs

The clinician edits the draft, corrects any code, and signs. Nothing is mapped to MatrixCare until that review is done.

4

Sync

Map into MatrixCare

AGIMAN maps the signed note and the approved ICD and CPT codes to the MatrixCare fields your facility uses, attaching to the resident or patient record through the access path MatrixCare approves. The export follows the same MatrixCare structure your team already charts in.

After sign-off

What AGIMAN hands back to MatrixCare.

Signed note

The reviewed chart note

The note the clinician signed, mapped into the resident or patient record fields MatrixCare expects for that care setting. AGIMAN does not write to the chart on its own.

ICD / CPT

Codes for the encounter

The ICD and CPT codes the clinician approved, attached to the encounter so billing and the record stay in step. AGIMAN scores 73.7 on a local medical benchmark, and a clinician confirms every code.

Documents

Letters, forms, and follow-ups

Referral letters, intake and visit forms, patient handouts, and the follow-up task and schedule note, prepared for the clinician to confirm and attach or send.

On your network

Local first, before any export.

  • Room audio and transcripts stay on the appliance on your network.
  • Transcription and note drafting run on site, with no internet connection required.
  • Nothing reaches MatrixCare until the clinician reviews and signs.
  • Exports use the access path your practice approves, with a BAA available.

Questions

MatrixCare, answered.

Where does resident audio and chart text live?

Room and home-visit audio, transcripts, and drafts stay on the AGIMAN appliance on your facility network. The only data that moves is the signed note and approved codes, and only over the export path your facility and MatrixCare approve. A BAA is available, and AGIMAN is HIPAA compliant.

How does AGIMAN reach MatrixCare?

Through the access MatrixCare approves for integrators, using open standards like FHIR R4, CCDA, and HL7v2 where the workflow supports them. MatrixCare reviews integrations through its Connections program and Partner Marketplace. We scope the export to the documents and codes you chart and test it against your own instance before go-live.

Does AGIMAN chart on its own?

No. AGIMAN drafts and suggests. A clinician reviews, edits, and signs every note and confirms every code before anything is mapped into MatrixCare. The clinical judgment stays with your team.
