Post-acute EHR integration

Draft the visit on the appliance, sign it into Netsmart myUnity

AGIMAN captures the visit, drafts the note in the clinician's style, and suggests ICD and CPT codes for behavioral health, home care, and hospice encounters. The clinician reviews and signs, then the result maps into the myUnity record through Netsmart CareConnect.

The connection

How AGIMAN connects to Netsmart.

What Netsmart is

Netsmart builds EHR software for post-acute and community care, including behavioral health, addiction treatment, home care, hospice, and human services. Its myUnity record runs the day for home health and hospice agencies. Netsmart operates CareConnect, the company's interoperability program, and a Qualified Health Information Network for nationwide exchange.

The access path

FHIR R4SMART on FHIROAuth 2.0HL7v2CCDA

AGIMAN connects through the Netsmart CareConnect FHIR R4 API, the documented standards path Netsmart exposes across myUnity and its other products, authorized with OAuth 2.0 and SMART on FHIR. AGIMAN maps the reviewed note and codes to the FHIR resources and myUnity fields the API approves, and reads patient and encounter context the same way. The first export path is scoped and tested against your own Netsmart instance before it is turned on, so the fields, the encounter type, and the codes match how your agency documents. This is not a certified partnership or an endorsement by Netsmart.

The Netsmart CareConnect export path is scoped and tested against your own myUnity instance before go-live.

The workflow

From the exam room to Netsmart, with you in control.

1

Capture

Record the visit on the appliance

AGIMINION captures the visit in the home, clinic, or facility, and audio and transcript stay on the appliance inside the agency network. No internet connection is needed to transcribe or draft.

2

Draft

Note and codes in the clinician's style

AGIMAN drafts the encounter note in the clinician's format and suggests ICD and CPT codes for the visit. It can also prepare a referral letter, an intake or visit form, and the follow-up task.

3

Review and sign

The clinician reviews and signs

The clinician edits the draft, adjusts the codes, and signs off. Nothing leaves the appliance until a person approves it.

4

Sync

Map into myUnity through CareConnect

The signed note and codes map to the FHIR R4 resources and myUnity fields the Netsmart CareConnect API approves for your agency. A review log records what was written and when.

After sign-off

What AGIMAN hands back to Netsmart.

Signed note

The encounter note

The reviewed and signed visit note maps into the myUnity record through the fields the CareConnect FHIR R4 API approves for your instance.

ICD / CPT

Codes for the encounter

Suggested ICD and CPT codes ride with the signed note as coded entries the clinician confirmed, mapped to the encounter Netsmart expects.

Documents

Referrals and forms

Referral letters, intake and visit forms, and patient handouts are prepared for the clinician to confirm before they are saved or sent.

On your network

Local first, before any export.

  • Room audio and transcripts stay on the appliance on your network.
  • Transcription and note drafting run on site, with no internet connection required.
  • Nothing reaches Netsmart until the clinician reviews and signs.
  • Exports use the access path your practice approves, with a BAA available.

Questions

Netsmart, answered.

Where does the visit audio and the draft live?

On the AGIMAN appliance inside your agency network. Room audio, transcripts, and drafts stay local, and transcription and drafting run with no internet connection. Only the signed result is exported, through the CareConnect access path your agency approves.

How does AGIMAN reach myUnity?

Through the Netsmart CareConnect FHIR R4 API, authorized with OAuth 2.0 and SMART on FHIR. AGIMAN maps reviewed text and codes to the FHIR resources and myUnity fields the API approves. It is the open standards path Netsmart documents, not a certified partnership or an endorsement.

Does AGIMAN write to the chart on its own?

No. A clinician reviews and signs every note and confirms the codes first. AGIMAN scores 73.7 on a local medical benchmark, ahead of nearby local-edge and small-cloud models, and a person still signs before anything reaches the myUnity record.
