Post-acute EHR integration

AGIMAN drafts the visit note and syncs it into WellSky

For home health, hospice, and personal care agencies on WellSky, AGIMAN records the home or facility visit, drafts the chart note in the clinician's style, and suggests ICD and CPT codes. The clinician reviews and signs, then the approved note and codes map to the agency's WellSky fields.

The connection

How AGIMAN connects to WellSky.

What WellSky is

WellSky is a post-acute and community care software company. Its products run home health (the former Kinnser), hospice and palliative care (Consolo), inpatient rehab, blood and biotherapies, and personal care agencies, and it processes a large share of Medicare home health claims. For many agencies it is the EHR of record for intake, OASIS documentation, scheduling, and episode-of-care charting.

The access path

FHIR R4US CoreSMART on FHIROAuth 2.0CCDA

AGIMAN connects through WellSky's standards-based interoperability, the WellSky I/O framework, which exposes FHIR R4 and US Core resources, SMART on FHIR access secured with OAuth 2.0, and CCDA documents across the post-acute products. WellSky also participates in CommonWell and TEFCA for nationwide record exchange. AGIMAN maps the reviewed note and codes to the fields the agency's WellSky instance approves. We scope and test that first export path against the agency's own instance before it is turned on, so the mapping is confirmed before go-live.

The WellSky export path is scoped and tested against your own WellSky instance before go-live, and is not a certified WellSky partnership or endorsement.

The workflow

From the exam room to WellSky, with you in control.

1

Capture

Record the home or facility visit

AGIMINION captures the visit audio in the home, facility, or clinic. The audio and transcript stay on the appliance on the agency network, with no internet connection required to transcribe.

2

Draft

Draft the note, suggest codes

AGIMAN drafts the chart note in the clinician's style and suggests ICD and CPT codes for the encounter. It can also prepare referral letters, patient handouts, and intake or visit forms.

3

Review and sign

Clinician reviews and signs

The clinician reviews every line, edits anything, and signs. Nothing reaches WellSky until a person has approved it.

4

Sync

Map into WellSky

The signed note and codes map to the agency's WellSky fields over its FHIR R4 and US Core resources, using the SMART on FHIR access the agency approved. A review log records what was sent.

After sign-off

What AGIMAN hands back to WellSky.

Signed note

The visit note

The clinician-signed chart note maps to the encounter in the agency's WellSky record over FHIR R4 and US Core resources.

ICD / CPT

Codes for the encounter

Suggested ICD and CPT codes travel with the note after the clinician confirms them, so coding starts from a reviewed draft rather than a blank form.

Documents

Referrals and forms

Referral letters, patient handouts, and intake or visit forms are drafted for the clinician to approve, then saved or attached through the access path the agency allows.

On your network

Local first, before any export.

  • Room audio and transcripts stay on the appliance on your network.
  • Transcription and note drafting run on site, with no internet connection required.
  • Nothing reaches WellSky until the clinician reviews and signs.
  • Exports use the access path your practice approves, with a BAA available.

Questions

WellSky, answered.

Where does the visit audio and the note live?

Room audio, transcripts, and drafts stay on the AGIMAN appliance on your agency network. No internet connection is needed to transcribe or draft. Only the approved note and codes leave the appliance, over the WellSky access path you turn on.

How does AGIMAN reach WellSky?

Through WellSky's standards-based interoperability: FHIR R4 and US Core resources, SMART on FHIR secured with OAuth 2.0, and CCDA documents. We scope and test the export path against your own WellSky instance before go-live. This is not a certified partnership or a WellSky endorsement.

Does AGIMAN write to the chart on its own?

No. The clinician reviews and signs every note and confirms the codes before anything maps to WellSky. AGIMAN removes the typing and the coding draft, not the clinical judgment, and a review log records what was sent.
