What WellSky is
WellSky is a post-acute and community care software company. Its products run home health (the former Kinnser), hospice and palliative care (Consolo), inpatient rehab, blood and biotherapies, and personal care agencies, and it processes a large share of Medicare home health claims. For many agencies it is the EHR of record for intake, OASIS documentation, scheduling, and episode-of-care charting.

The access path
AGIMAN connects through WellSky's standards-based interoperability, the WellSky I/O framework, which exposes FHIR R4 and US Core resources, SMART on FHIR access secured with OAuth 2.0, and CCDA documents across the post-acute products. WellSky also participates in CommonWell and TEFCA for nationwide record exchange. AGIMAN maps the reviewed note and codes to the fields the agency's WellSky instance approves. We scope and test that first export path against the agency's own instance before it is turned on, so the mapping is confirmed before go-live.

Review and sign
The clinician reviews every line, edits anything, and signs. Nothing reaches WellSky until a person has approved it.
Sync
The signed note and codes map to the agency's WellSky fields over its FHIR R4 and US Core resources, using the SMART on FHIR access the agency approved. A review log records what was sent.
After sign-off
The clinician-signed chart note maps to the encounter in the agency's WellSky record over FHIR R4 and US Core resources.
Suggested ICD and CPT codes travel with the note after the clinician confirms them, so coding starts from a reviewed draft rather than a blank form.
Referral letters, patient handouts, and intake or visit forms are drafted for the clinician to approve, then saved or attached through the access path the agency allows.
